Échec D’extubation En Réanimation Chirurgicale Dans Un Contexte De Ressources Limitées A Madagascar
Abstract
Méthodes : Une étude prospective cas-témoins a été menée dans le service de réanimation chirurgicale du Centre Hospitalier Universitaire Professeur Zafisaona Gabriel de Mahajanga entre septembre 2016 et septembre 2019. Les patients intubés pendant plus de 24 heures puis extubés ont été inclus. L’échec d’extubation était défini par la nécessité d’une réintubation dans les 48 heures suivant l’extubation. Pour chaque cas, deux témoins présentant une extubation réussie ont été sélectionnés.
Résultats : Parmi les 173 patients inclus, 50 ont présenté un échec d’extubation, soit une fréquence de 28,9 %. Les facteurs associés à l’échec d’extubation étaient l’âge avancé, l’hypertension artérielle, la neurochirurgie, la chirurgie urgente, une durée opératoire prolongée, une intubation supérieure à deux jours et la présence d’un état de choc avant l’extubation. Les principales causes de réintubation étaient l’altération neurologique (46 %), la détresse respiratoire aiguë (28 %) et l’encombrement bronchique (26 %). La mortalité chez les patients présentant un échec d’extubation était de 30 %.
Conclusion : L’échec d’extubation était fréquent dans notre contexte. Les défaillances neurologiques et respiratoires constituaient les principales causes de réintubation. Une meilleure évaluation pré-extubation pourrait contribuer à réduire cette complication dans les contextes à ressources limitées.
