Gestion Hospitalière Au Sein Du Centre Hospitalier De Reference De District D’anosy Avaratra

Abstract


Cette étude évalue la performance de la gestion hospitalière au sein du CHRD Anosy Avaratra. L’objectif a consisté à analyser la qualité de la gestion hospitalière à travers le modèle de Donabedian (structure, processus, résultats) afin d’identifier les leviers d’amélioration de la gouvernance et de la performance. Il s’agit d’une étude évaluative, descriptive, rétrospective et transversale qui a été menée de 2022 à 2024, à partir de données documentaires, d’entretiens et de questionnaires adressés au personnel hospitalier. Les indicateurs retenus concernent la planification, les ressources humaines, le financement et la satisfaction. En effet, malgré la fonctionnalité des comités de gouvernance et la planification régulière, l’étude révèle un paradoxe entre la gestion administrative formelle et la gestion opérationnelle réelle. Le taux d’exécution budgétaire est tombé à 7,25 % à mi-2024, alors que l’activité hospitalière a progressé de 21,58 % et les recettes propres de 15,36 %. Ces déséquilibres entraînent la rupture des soins, la détérioration du capital humain et une gestion de survie fondée sur les recettes propres. Afin d’apporter une amélioration à la gestion de l’hôpital, il est suggéré de renforcer la planification et la gestion budgétaire, enrichir les ressources humaines et améliorer la gestion des informations.
